HCP Talent is collaborating with a Long-Term Care/Rehabilitation (280 beds) in Arverne, NY 11692 that is seeking a Social Work Director (MSW) for a full-time Day shift (7a-3p) M-F position. SW Duties and Responsibilities: Ensure that all Social Work documentation is completed in accordance with facility policy and procedure as applicableCoordination of Interdisciplinary Discharge Planning (Knowledge of community resourcesDemonstrate compliance with government and accrediting agency standards and regulations pertaining to Social WorkInterface with facility departments and outside agencies in order to provide a complete plan of care and to facilitate services. Liaison between family and other interdisciplinary team membersBenefits: Day shift (7am-3pm) Monday-FridayCompetitive Compensation: up to $110K per year 401K with 3% matchingA TON OF PTO: 4 weeks vacation, 15 sick days, 11 holiday days and 3 personal days.
