About the Opportunity:Schedule: Monday to FridayHours: 8am to 5pmSetting: ClinicPatient Population: 65+Patient Volume: 12 per dayResponsibilities:
The Nurse Practitioner will be responsible for:Provide patient facing ACUTE and other medical evaluations to individual membersEncounters for assessmentPost-hospital discharge stabilization, and follow up and consultation with stabilization coordinators for state-wide panel of members. Evaluate and document accurate patient information to provide expert health assessment and treatment of complex conditions via telehealth platform including interpretation of vitals, medication reconciliation!Coordination with primary care providers, specialists, and hospital-based providers. Qualifications:Licensed to practice medicine in the State of TexasRequired Certifications Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC,)Your privacy is important to us.
